The development and implementation of information systems in the field of medical technology is a rather urgent task. Analysis of the use of personal computers in medical institutions shows that computers are mainly used for processing text documentation, storing and processing databases, and statistics. Some of the computers are used in conjunction with various diagnostic and therapeutic devices. In most of these areas of use the computers use standard software – text editors, DBMS, etc. Therefore, the creation of an information organizational and technical system capable of establishing a patient’s diagnosis in a timely and reliable manner and choosing an effective treatment tactic is an urgent task of informatization.

The task of diagnostics in the field of medicine can be put as finding the relationship between symptoms (input data) and diagnosis (input data). For realization of effective organizational and technical system of diagnostics it is necessary to use methods of artificial intelligence. The feasibility of such an approach is confirmed by the analysis of data used in medical diagnostics, which shows that they have a number of features, such as

qualitative nature of the information, the presence of data omissions: a large number of variables with a relatively small number of observations. In addition, the considerable complexity of the object of observation (diseases) often does not allow to construct even a verbal description by the doctor of the diagnosis procedure. Interpretation of medical data resulting from diagnosis and treatment is becoming one of the serious areas of neural networks. At the same time, there is a problem of their correct interpretation. The wide range of tasks, solved by neural networks, does not allow to create universal powerful networks so far, forcing to develop specialized neural networks for solving complex tasks of medical diagnosis is: the lack of necessity of explicit mathematical modeling and validity check of serious assumptions for using statistical synthesis method from dimensionality of space, signs of neural networks, etc.

However, the use of neural networks for medical diagnosis tasks is also associated with a number of serious difficulties. These include the need for a relatively large sample size for network tuning, the focus of the mathematical apparatus on quantitative variables.

Systems for monitoring

The task of real-time assessment of a patient’s condition arises in a number of very important practical fields in medicine and, first of all, in case of continuous monitoring of a patient in intensive care wards, operating rooms and post-operative wards.

In this case it is required to provide not only operative diagnostics of complications during treatment, but also prognosis of the patient’s condition, as well as to determine optimal correction of arising disorders on the basis of long-term and continuous analysis of a large volume of data, characterizing the state of physiological systems of the organism. To solve this problem, monitor MSCs are designed. The most frequently used monitoring parameters include: electrocardiogram, blood pressure in different points, respiratory frequency, temperature curve, blood gas content, minute blood circulation volume, gas content in exhaled air.

The hardware of monitor systems and similar systems for functional diagnostics practically does not differ in principle.

An important peculiarity of the monitoring systems is the availability of express-analysis means and visualization of the results in real-time mode. It allows displaying on the monitor screen also the dynamics of various derivatives of the controlled values. All this is done in different time scales. The introduction of IT in healthcare began relatively recently, with the emergence of the first medical information systems (MIS). Digitization of materials was and still is the most popular technology. Paper charts, prescriptions, “coupons”, cardiograms, scans – everything went into electronic form.

The information obtained had to be structured in such a way that it could be handled by anyone who needed it. Databases and database management tools (DBMS) appeared – the interface linking the data operator (user) and his functions with the database.

The accumulation of large amounts of data and the ability to isolate and verify the relationships between them – for example, between the nature of the disease and medication prescription – made it possible to apply decision support systems in medicine. These same conditions make the development of expert systems, artificial intelligence, and machine learning possible.

Applied IT solutions in medicine

In addition to the information component, IT is also about the speed of transmission of digital materials in any geographic location. This is the basis for the use of telemedicine systems, a complex of audio-visual equipment which is popular today and which can organize real-time reception even when a doctor and a patient are separated by long distances.

The development of technology is largely determined by its cost. The more expensive a technical solution is, the narrower the range of its application. Therefore, when innovations enter the broad market, there is a real boom in their use. Previously, personal monitoring of vital indicators of the human body, such as heart rate (HR) and blood pressure (BP), was carried out only at the time of going to the doctor or independently – with prolonged physical discomfort. Now the blood pressure monitor and heart rate monitor are built into virtually every smart watch model, and with the help of accompanying applications, it is possible to track the dynamics of these indicators over the entire time the device is worn and under different loads.

Of course, now such know-how is used mostly for personal monitoring, but the day is not far off when all the collected indicators will be automatically uploaded to a personal cloud-based health record, analyzed by the system’s smart algorithms, detecting abnormalities in heart and vascular function and signaling them – all without direct human intervention.

However, something similar is already happening, but within medical institutions. This phenomenon is called the medical Internet of Things. Sensors monitoring various patient indicators collect and transmit information to central monitoring “nodes” without the participation of medical staff. At the same time, the sensors “understand” which dynamics of the condition are negative and can report this.

As a result, bioengineering became a real necessity in improving health-related services. Regarding the growth of technology in Brazil, it is known that there was a growth of postgraduate courses in biomedical engineering after 1990, where there was a real structuring of the topic.

Despite the barriers, the future of electromedicine is promising, as major projects are being developed in fields of engineering related to neuroscience, in addition to bioinformatics, biomechanics, including, as seen in the norms visualized by the Regional Medical Council of the State of São Paulo in its resolution 097. The methods used are becoming more and more advanced every day, and the field of neural engineering has advanced considerably as it receives support from several sectors. In this sense, an electrical stimulator is being fixed in the brains of people suffering from Parkinson’s disease in order to better observe brain function. Therefore, the advances are notorious and increasingly innovative. However, professionals must be aware of the possible errors in the execution of these types of equipment, as contact different types of losses and waste of different equipment, due to the lack of trained professionals. In this sense, the profile of the biomedical engineer must be creative and motivating, because an effective and competent professional will work both in hospitals and companies, as well as in the development of new electronic projects aimed at health. Another area that opens the door to electromedicine concerns universities and academic research through large centers and public universities, as well as private institutions, where productivity is carried out in bioengineering projects using tools such as prostheses, orthopedic insoles, and artificial organs.
